At the bottom of the page you will find the current materials release forms for all of my upcoming projects. I will need this filled out so that I can use your comment/photo/video in the film.

Here is a short list of instructions to help you fill it out.

1) At the top in 'From' section, please fill in your full name.

2) In section "1" of the release there is a blank space on the second line for you to fill in the details of what you are giving me the rights to use.
The files need to have the exact name that you include on the form. I suggest
renaming the files before listing them so they are't just a long list of numbers.

For example, you could change the name of the files to:
Emilysmithvideo1.mov, Emilysmithvideo2.mov etc.
(the number asscending etc, this also allows me to find who gave me the
video at a later date if I have any further questions)

If you are sending several files and their names won't fit in the space provided,
please write in the space, "see bottom of form" and add the names of the files
below the area for signatures etc.

Please only send files that you own the rights to.


3) At the bottom of the page there is a space for a signature,
date and for you to print your name.

4) Please send the photos/videos over to me using Wetransfer, Dropbox or a similar
platform that won't compress or degrade the quality of the media.
Please include thecompleted release in with the file(s).
